Bundled Wraparound Fidelity Assessment System and WrapStat

The Wraparound Fidelity Assessment System ("WFAS") is a multi-method approach for assessing the quality of individualized care coordination for children and youth with complex needs and their families.

UW’s Wraparound Evaluation and Research Team ("WERT") seeks to improve the lives of children and their families by conducting research on and supporting continuous quality improvement for Wraparound care coordination. This includes development and dissemination of the WFAS instruments, including:

•    WFI-EZ – the Wraparound Fidelity Index-Short Form
•    TOM 2.0 – Team Observation Measure, version 2.0
•    DART: Document Assessment and Review Tool; and
•    WrapStat: the WFAS Data Entry, Management and Reporting system.

Collectively, these tools ("WFAS Tools"), are licensed together herein in collaboration with the University of Illinois to serve participating Care Coordination & Support Organizations ("CCSOs") within the state of Illinois.

Please note: Only those Organizations that are confirmed by UW and UConn to be a “Participating NWIC Organization” are eligible to use this Agreement.  Please review Exhibit F – List of Participating NWIC Organizations or contact the WERT team at wrapeval@uw.edu as needed to confirm your Organization's eligibility prior to submittal.
