Care Management Tracking System (CMTS)
The Care Management Tracking System (CMTS) is a HIPAA-compliant web application that supports the core principles of effective integrated care. It is ideal for research studies or contexts that span multiple healthcare organizations with specific tracking/reporting needs. More information about CMTS can be found on the AIMS Center’s website. Healthcare organizations must sign a license and make UW their business associate in order to use CMTS.
New York State Collaborative Care Learning Network and Collaborative Care Medicaid Program CMTS & Data Storage License Agreement
This license is only available to designated organizations participating in the New York State Collaborative Care (CoCM) Learning Network OR the New York State Collaborative Care Medicaid Program.
Only those organizations that are confirmed by the New York Office of Mental Health (OMH) to be an authorized organization participating in one of these programs are eligible to use this agreement; first year hosting fees described herein are paid by NY OMH. If you are unsure if your organization qualifies, please contact AIMS Center at firstname.lastname@example.org to confirm.
