PainTracker

PainTracker™ is a University of Washington quality improvement tool that collects pain-related data to help clinicians and patients track and improve the core outcomes of chronic pain management. PainTracker is currently only available as a printed form and as a demonstration test-account for an online web-hosting program.

International Requests:

University of Washington agreements require additional review if the requesting entity is located in, or affiliated with the government of, China (Including Hong Kong; not including Taiwan), Iran, North Korea, Russia, or Syria. For requests from these countries, please allow for an additional month of processing time for a response.
